FOR HEALTHCARE PROVIDERS

Partnering with referring providers across Southwest Missouri.

Affect Health works alongside primary care, pediatrics, emergency medicine, hospital medicine, and community partners to deliver responsive, evidence-based psychiatric care for the patients you refer.

APPROPRIATE PATIENTS

Who to refer to Affect Health.

We accept adults and adolescents needing outpatient psychiatric evaluation, medication management, and specialty behavioral health services. If you're unsure whether a patient is a good fit, we're glad to talk it through.

  • Adults and adolescents needing comprehensive psychiatric evaluation
  • Patients requiring ongoing medication management for mood, anxiety, ADHD, or trauma-related disorders
  • Treatment-resistant depression candidates who may benefit from Spravato® (esketamine)
  • Individuals with substance use disorders or co-occurring mental health and addiction conditions
  • Patients seeking thoughtful, collaborative deprescribing of psychiatric medications
  • Diagnostic clarification when symptoms overlap or response to prior treatment has been limited

DIRECT REFERRALS

A simple, streamlined referral process.

We designed our intake to minimize friction for referring providers and their patients — clear expectations, prompt follow-through, and quick scheduling.

01

Send the referral

Fax or securely message the referral. Please include reason for referral, current medications, relevant psychiatric and medical history, and recent labs when available.

02

We review & schedule

Our team reviews the referral, contacts the patient to schedule, and confirms insurance and intake paperwork prior to the first visit.

03

Warm hand-off

New patients are typically seen within 2–3 weeks. Urgent referrals are triaged the same business day when possible.

CARE COLLABORATION

What referring providers can expect.

Our practice is built around collaborative care. You stay informed, your patient stays supported, and treatment decisions are made together.

Timely consult notes

You will receive an initial consultation note after the first visit summarizing diagnosis, treatment plan, and medication changes.

Ongoing communication

We share progress updates at meaningful clinical milestones and any time medication changes affect co-managed care.

Shared treatment planning

For patients you continue to see, we coordinate on labs, monitoring, and adjunctive medical concerns so care stays aligned.

Clear scope & hand-offs

We communicate clearly when a patient is stabilized and appropriate for return to primary care, or when a higher level of care is indicated.
